The pasuk of ולא תשקצו teaches us that one should not make oneself disgusting, included in this is that in general one should not hold back from relieving oneself when necessary. However, not every time one feels the need to relieve oneself do they need to. For example, one who is teaching in public is not required to interrupt in order to relieve oneself, even if one is under immediate pressure.
